COPD

The Caring for My COPD Program offers support to clients with a confirmed diagnosis of Chronic Obstructive Pulmonary Disease (COPD).  The 10-week program supports clients with understanding their chronic disease and how optimizing both physical and emotional health is key to managing their breathing.  Our team works collaboratively to ensure clients are provided education, exercise, and social support to effectively manager their COPD.

What our clients can expect:

  •          Weekly group education sessions to help clients better manage their COPD
  •          One-on-one support from members of the Care Team
  •          Group and individual exercise sessions
  •          Holistic wrap-around care and access to community programs, services, and resources

There is NO cost to this program and there is free parking onsite!

Diabetes Team

Our team of Certified Diabetes Educators in the program includes Registered Nurse, Registered Dietitian and Pharmacist. We will be able to assist you on your diabetes journey (wherever you are in it).

Caring for Your Diabetes

These are a series of group classes offered onsite throughout the calendar year.

  •          Head to Toe
  •          Learn why Food and Movement Matter
  •          Insulin- New to Insulin or a Refresher
  •          Caring for your Diabetes – Health Promotion: for Farsi speaking clients. This group meets once a month.

Diabetes Kitchen

Diabetes cooking program where participants will cook a balanced meal and learn how to save money at the grocery store.

Primary Care Asthma and COPD Program (PCAP)

The Primary Care Asthma and COPD Program (PCAP) program exists to help patients manage their Asthma and/or COPD symptoms and control by reviewing things like inhaler technique, avoiding triggers and better breathing.  The program also helps patients who smoke cigarettes, marijuana, and/or vaping to quit or cut down through free (6 months) nicotine therapy such as the patch, inhaler, gum, and lozenge.  PCAP also helps doctors and nurse practitioners (NP) identify whether patients have Asthma or Chronic Obstructive Pulmonary Disease (COPD) through a special breathing test called Spirometry, as well as gives them occasional updates on innovations and information on lung health.

Summary of program:

  • Spirometry testing before and after a rescue inhaler to determine whether a patient has Asthma or COPD.
  • Patient education on Asthma, COPD, Sleep Apnea including puffers, triggers, devices, breathing, self-management care.
  • Doctor or NP resources and updates on lung health, best practices, etc.
  • Research and innovation developments in lung health pertaining to Asthma or COPD.
